Alex Daniel Chuculate, 27, passed from this life on July 8, 2021, in Tulsa, OK. Alex was born on March 17, 1994, in Ft. Smith, AR to Jerry D Chuculate and Stephanie Cogburn. Alex worked for Fed Ex on the weekends as a package handler and delivery, also working for Turner Roofing Co. in Tulsa.
Alex is survived by his parents, Jerry and Stephanie Chuculate of Sallisaw; his children, Emery Danielle Chuculate, Benjamin Taylor Chuculate, and Greyson Michael Lee Jones; siblings, Chealsie Cogburn of Tulsa, Aubrie Shackelford of Ft. Smith, Sara Starks of Sallisaw, Beth Chuculate of Sallisaw, and Amber Chuculate of Sallisaw.
Visitation for Alex will be held on Sunday afternoon, July 18, 2021, from 2:00 pm to 5:00 pm in the Sequoyah Memorial Chapel of Forever Memories Funeral Services, 463505 Hwy101, Sallisaw. Funeral services will be held on Monday, July 19, 2021, at 10:00 am in the Sequoyah Memorial Chapel with Mr. Heath Wood officiating. His final resting place will be in the Peters Cemetery, Sallisaw. Casket bearers will be Tyler Qualls, Larry Johnson, Paul Gibson, Jerry Chuculate, Derick Johnson, and Robert Childers.
